Directions:
1. Print off BOTH the Activity Application and the Participant Agreement forms.
2. Read and sign both forms. Be sure your daughter signs it, too.
3. Mail the forms to:

OR
bring the forms with you on the first day of the Club or Camp
REMEMBER: In order to be fully registered, you need to turn in both of these forms AND have filled out the on-line registration form.
